What is CMARC (Care Management for at Risk Children Program)?
The CMARC program is an early childhood program that focuses on improving health care outcomes for young children. Care managers coordinate services with medical providers to ensure appropriate health and medical care, developmental and social supports are obtained to increase the quality of care provided and decrease the cost.
Who is eligible for CMARC?
Children residing in Cumberland County, ages birth to five years old with a medical, mental or behavioral healthcare condition or diagnosis. Those include NICU graduates, significant or chronic health care needs, children exposed to adverse childhood experiences that could result in unmanageable stress, foster care and those in need of developmental assessments or services.
What can you do for me?
We recognize you as the expert on your child. With your help, we will assess your child and family strengths and needs, determine goals, work with you in establishing a strong link to a medical home (regular doctor), other medical specialist, provide growth and developmental screenings, discuss and offer referrals to community programs and supports to deal with challenging levels of stress.

Free to all participants who are eligible.
